Problem 4: I added javascript logging to my Reference Qualifier javascript method, and it seems like it's actually getting called when I change Provider, but the listings for Device and Plan do not update.
Idea 5: I had a brainstorm. Many of the forums I've seen about dynamic Reference Qualifiers talk about Reference forms. I'm wondering if I simply cannot use Lookup Select Boxes, and have to change the form to Reference input boxes.
